CHN Health Management Program - The Process

The Process

STEP1:
Health Evaluation

  • Health Questionnaire and Biometric Screening
  • "High Risk" Individuals Identified
  • Health Status Report - 30 minute session with Health-Y-Coach
  • Organizational Aggregate Report

STEP 2:
Weekly Onsite Coaching

  • One-on-one meetings with Health-Y-Coach to develop an individualized plan to manage health risks which include:
    • Nutrition & weight management
    • Stress management
    • Exercise
    • Tobacco cessation
  • Group programs to address the issues of weight loss, tobacco cessation, and diabetes management
  • "High Risk" employees are retested and provided an Outcomes Report

STEP 3:
Health Education and Promotion

  • Ongoing coaching
  • Health-Y-Trax Web site
  • Monthly Healthy-Y Break
  • Quarterly competitions and challenges

Before diseases can be prevented, risks for those diseases must be identified and the risks explained to the employee. Most employees, even if they go to the doctor regularly, do not understand the risks of chronic disease.

The CHN Health-Y-Coach is a 4-year college degreed health professional, trained to build strong relationships with individuals and to use the tools and protocols CHN has developed to enhance behavior change. Each employee works with a coach to design a program specific to that individual, from which weekly goals and action plans are assigned.

Health improvement of the entire work force is the goal of the CHN Health Management Program. All employees, regardless of risk level, are encouraged to participate in activities to promote health, education, and to make healthy lifestyle as much fun as possible.
